The scenario is that a donor in the community wants to give $1,000,000 to address a priority health need or issue in the area. As a community health Planner, you have been asked to write a 3-5 page double spaced proposal to make a case for your assigned community to get the funds. Your Professor has specifically assigned you a Community Health Needs Assessment . The CHNA identifies the priority health needs of a specific geographic area. You are to develop a proposal for getting the funds from this concerned donor.

To avoid plagiarism, all content that is incorporated into the analysis needs to be adequately sourced and include a reference page if it is pulled from external sources or is not common knowledge.

The paper needs to include the following. For easy readability, answer each question in a separate section within the paper.

1. An overview of the demographics (age, race, gender) and health needs for your specific

assigned community.

2. Which health disparity or disparities will your proposal focus on and why? Only pick a

maximum of 2 health disparities within the community. Compare the health disparity

statistics to either the state or county, in order to show if the prevalence rate is higher or

lower compared to your assigned community.

3. For each health disparity that your paper will focus on (maximum of 2), develop a

strategy to include the 4 steps listed below. Be specific on how you plan on addressing each health disparity.

a. What will be done?

b. Who is going to be the lead and work on it?

c. When will the implementation of the strategy occur?

d. Develop a Metric to measure the anticipated targeted improvement compared to the baseline (if available).

4. Determine what success will look like if you are given $1,000,000 and how long are you anticipating the money will last? 6 months, 1 year? 3 years?

5. What financial controls will be put in place to ensure the donated money is being used appropriately to minimize fraud?

You must proofread your paper. But do not strictly rely on your computer’s spell-checker and grammar-checker; failure to do so indicates a lack of effort on your part and you can expect your grade to suffer accordingly. Papers with numerous misspelled words and grammatical mistakes will be penalized. Read over your paper – in silence and then aloud – before handing it in and make corrections as necessary. Often it is advantageous to have a friend proofread your paper for obvious errors. Handwritten corrections are preferable to uncorrected mistakes.

Use a standard 10 to 12 point (10 to 12 characters per inch) typeface. Smaller or compressed type and papers with small margins or single-spacing are hard to read. It is better to let your essay run over the recommended number of pages than to try to compress it into fewer pages.

Likewise, large type, large margins, large indentations, triple-spacing, increased leading (space between lines), increased kerning (space between letters), and any other such attempts at “padding” to increase the length of a paper are unacceptable, wasteful of trees, and will not fool your professor.

The paper must be neatly formatted, double-spaced with a one-inch margin on the top, bottom, and sides of each page. When submitting hard copy, be sure to use white paper and print out using dark ink. If it is hard to read your essay, it will also be hard to follow your argument.
