Preparation This intervention would take effect with proper ED discharge training. The doctors, mid-levels and nurses will be briefly educated on appropriate discharge procedures before starting with the discharge checklist tool intervention. The training will be guided by researcher and consists in introducing a discharge checklist tool and how to use it appropriately when discharging a patient from the ED. The doctors, mid-levels providers and nurses will participate in this training. A pretest will be given just to have an idea on how each healthcare provider takes care of a discharge. Additionally, a post test will be provided at the end of the training. The checklist tool will assure that patients understood all information provided and resources were given before being released from the emergency department. The patient and healthcare provider will need to sign affirming discharge instructions were cleared and patient is ready for check out. This tool will be place in patient’s medical records. The expected outcome and evaluation of this activity is to ensure that healthcare providers have necessary tools to discharge a patient appropriately from the emergency department by providing high quality of care. The effectiveness of this intervention will be manifested in the improvement of service delivery.
